Terms & Conditions

We understand that life happens. If you need to cancel or reschedule an appointment, please provide at least 24 hours advance notice by calling our office at (918) 748-3640 or emailing support@fmidr.com.

Patients who miss scheduled appointments without advance notice will be charged a $75.00 no-show fee. This fee is non-refundable and must be paid before your next scheduled visit.

Repeated no-shows (two or more within a 12-month program period) may result in a formal review of your program enrollment and, in some cases, discontinuation of services.

All fees for services already rendered are non-refundable, including completed consultations, lab orders already submitted, prescriptions written, and administrative services.

For pre-paid program services not yet rendered, refund requests must be submitted in writing within 30 days of enrollment to support@fmidr.com. Approved refunds will be calculated based on the remaining unused portion of your program, minus any services already delivered.

A processing fee of 2.9% + $0.30 will be deducted from all credit or debit card refunds to cover non-recoverable payment processing costs. ACH bank transfer refunds are not subject to this fee.

No refunds will be issued for administrative fees, initial consultation fees, or laboratory services already ordered regardless of whether results have been reviewed.

By enrolling in a Functional Medical Institute wellness program, you acknowledge and agree to the following:

• You have reviewed your program proposal and understand the services included.
• You understand the 12-month commitment and the associated financial terms.
• You agree to make payments as scheduled per your selected payment plan.
• You understand that FMI provides functional and integrative medical services and is not a replacement for emergency care.
• You consent to electronic communications from FMI regarding your care and program.
